A union official or an employee of a union needs a permit to enter a premises. After the proposed permit holder completes training, apply for an entry permit using Form F42.

Who can use this form
An entry permit application is made by the union on behalf of the permit holder.
Only use Form F42 if you are:
- a member of the committee of management of a union AND
- applying for an entry permit for a union official (including an employee or elected officer of the union).
Only use Form F42 if you are applying for a new entry permit, or if the previous permit has expired.
If you are applying to renew a current entry permit you should use Form F42D – Application for renewal of an entry permit.
Before you start
Form F42 is made up of 4 parts:
- Information about the applicant union (organisation and branch if applicable).
- Information about the proposed permit holder.
- Declarations by the proposed permit holder and the committee of management member of the applicant union, stating that they have provided true and correct information about whether the proposed permit holder is a fit and proper person, and providing evidence that right of entry training has been completed.
- Declarations by the proposed permit holder, and by the committee of management member of the applicant union, stating that they have provided true and correct information about the proposed permit holder’s identity including signature and full-face, passport-sized photo attachment.
Have this information ready before you begin the application. You will need to give us:
- the permit holder's legal name and job title (or their office if they are an elected officer)
- the address we should send the permit to
- the name of the committee of management member making the application
- your name as the contact for the application (if different from the committee of management member)
- a declaration stating that the proposed permit holder is a fit and proper person to hold a permit
- evidence that the permit holder has completed appropriate right of entry training
- a recent full-face, passport-size photograph of the proposed permit holder.

Warning about providing false or misleading statement or information
A person who in a permit application:
- knowingly or recklessly makes a false or misleading statement or leaves out information (omits matters) which makes the statement misleading, or
- knowingly gives false or misleading information or omits a matter to make the information misleading,
may be found guilty of a criminal offence punishable by imprisonment. See sections 136.1 and 137.1 of Part 7.4 of Chapter 7 of the Criminal Code Act 1995.

What happens next
First-time permit holders must attend an identity verification meeting. We will send the proposed permit holder an invitation to the meeting by email.
The meeting will be held online using Microsoft Teams, and usually takes about 5 minutes. You should have your current driver’s licence, passport or ImmiCard ready so we can verify your identity in the meeting. The meeting will be recorded.
