Change of name on the current entry permit
If you have recently changed your name and your entry permit is current, we can issue an entry permit in your new name with the same permit number and the same expiry date.
This is a variation of the current entry permit and only requires you or your union to send us an email with a statutory declaration by you, and documentary evidence supporting the change of name (e.g. a copy of a driver’s licence in the new name, certificate of marriage, etc.) requesting the current permit be varied showing the new name.
Change of name but the current entry permit in the old name will expire soon, or has expired
If you have recently changed your name and the current entry permit will shortly expire, or has already expired, a new entry permit showing your new name with a new permit number and new expiry date may be issued.
If the current entry permit expires in less than 12 months, your union should lodge a Form F42D and a statutory declaration by you and documentary evidence supporting the change of name.
If the entry permit in the old name has already expired, your union must lodge a Form F42 and a statutory declaration by the proposed permit holder and documentary evidence supporting the change of name.
