Only use Form F86G if you are the regulated host covered by a regulated labour hire arrangement order.

Before you start
Have this information ready before you start the Form F86G. You will need to tell us:
- your contact details
- your representative’s contact details (if you have one)
- details of the regulated labour hire arrangement order to be varied
- details of the new employer or employers, relevant regulated employees and employee organisations
- information about how the additional employer or employers supply employees the regulated host, and the work being performed
- whether the new employer or employers agree to us varying the order to cover them, and
- if a new employer has not agreed us varying the order, additional information about the relevant regulated employees and the work they perform.
If a new employer agrees to us varying the order, ask them to complete section 4 of the application form before you lodge.

What happens next
You must send (serve) a copy of the application on:
- each new employer
- each employer already covered by the regulated labour hire arrangement order, and
- each employee organisation that can represent a regulated employee or an employee of the regulated host (that you know of).
We will email you to confirm we have received your application.
Applications are dealt with by a Member who may then organise a hearing or conference.
