Who can use this form
Fill in parts 1, 2 and 4 if you are an employer (or are completing this form for an employer) that has lodged a Form F23D and you need to lodge a declaration in support of that application.
OR
Fill in parts 1, 3 and 4 if a union has lodged a Form F23E and you need to lodge a declaration in relation to that application on behalf of an employer.
Before you start
Have this information ready before you begin. You will need to tell us:
- any material you gave employees about the variation of the agreement
- details of the voting process including dates, steps in the process
- the varied agreement and how you explained the terms of the agreement to employees
- information you gave employees about when and how to vote
- whether the varied agreement will cover general building and construction work
You must send (serve) a copy of your declaration to each employer and employee organisation that is covered by the agreement.
