Who can use this form
Use this form if you are:
- a registered organisation (like a union), or
- a regulated business
that signed the collective agreement.
Before you start
Have this information ready before you start.
You will need to give us:
- contact details for the negotiating entities that signed the collective agreement, and
- basic details of the collective agreement, such as its name and the class of regulated workers it covers.
When you submit this application form, you will also need to provide us with:
- a signed copy of the collective agreement, and
- Form F95 – Declaration in support of an application to register a collective agreement.

- Form F95 declaration
You can also send or deliver your application to the Commission office near you.
What happens next
As soon as possible after you lodge the application, you must send (serve) a copy of this application and any accompanying documents on the other negotiating entity that signed the collective agreement.
We will undertake an assessment of the collective agreement on the information and material provided.
